Client Forms To Complete

If you are a new client or haven't received any Skin Sense treatment in the past 6-months, we ask that you please print and fill out these forms before your scheduled appointment:

If you are a minor, under the age of 18, and are receiving any treatment(s) you will need to bring the following sheet with you:

To ensure that Skin Sense provides the appropriate treatment to each of our pregnant clients (must be 12+ weeks), we ask that you fill out our Pregnancy Consent forms:

*If you are under the care of an oncologist, please provide a medical clearance note or form from your doctor upon arriving for your treatment.

Appointment Requirements

  • We ask that you arrive 15-30 minutes early to fill out updated forms, change into a spa robe for your comfort, and enjoy our amenities.

    • If you are a new client or have not received a treatment in the past 6-months, we ask that you fill out updated treatment forms (see above for details.)

  • We ask that guests update their spa profile with the correct credit card information before your appointment in order to make your checkout process as easy as possible.

  • Please be aware of our 24-hour cancellation policy. You will be charged the full price of all scheduled treatments if you do not appear for your appointments, if you cancel less than 24 hours before your first appointment, or if you arrive late. You may reschedule or cancel more than 24 hours in advance by calling us at (919)870.07000. You will not receive a telephone call appointment reminder.  We send reminders only by email and/or text message. Should you or a family member test positive for Covid-19 and are unable to cancel within a timely manner, you will be required to have a doctor’s note.

  • Should you have a fever of 100.4 or greater, we will need to reschedule your appointment. You will need to be fever-free for three days with no respiratory symptoms for the integrity of all guests and staff alike.

  • We ask that you do not bring anyone with you to the spa without an appointment unless they are shopping in our boutique area.
